Please find links to The Nurse’s Office patient forms below. If you are a new patient of The Nurse’s Office and wish to complete your New Patient Paperwork prior to your first visit, please print, complete, and bring the forms with you to your first appointment. While it is not necessary to have your new patient forms with you, it may save time in the office during your first appointment. If you are a new pain management patient, please print, complete, and bring the New Patient Paperwork for Pain Management Patients form, if you wish to take care of your paperwork in advance of your first appointment.
For all of our patients, new and current, please find our Notice of Privacy Practices (HIPAA) linked below. This discusses how we handle your personal protected information, such as name, date of birth, social security number, and medical history information under the law. This applies to all of our patients, and you may refer to it at any time.
If you are a new patient to our office and wish to share your past medical information with your new provider, please print, complete, and submit the Authorization for Release of Medical Information form. If you are a current patient of The Nurse’s Office and wish to share your medical records from specialists or other members of your healthcare team, please also print, complete, and submit this form, so that we may have access to your medical information.
